I have a habit app that I started using a few weeks ago and found it really works for me. It puts just enough pressure on me to complete my list of 7 habits that I want to do almost everyday. I get to check them off along the way. Three of the habits are workout/strengthen, yoga/stretch and Physical Therapy stuff I am supposed to do regularly. Others are work related and hydration related. I was surprised this works. I suggest giving it a try. Keep the habits small and manageable, include things like strength and do the 7 strength exercises we do at practice. Also include sleep. Check off when you get 9-10 hours of sleep a night. Middle schoolers still need at least that much sleep. Cardio such as RUNNING should be included. Have a goal of 5-6 days a week hitting every habit. Good luck!
